Brief Description: 

Provides basic health cover for secondary and tertiary care up to Rs. 5 lakh per annum per family

How to apply:

 Beneficiary to submit application in Form B; Form B is verified by Nodal Officers/BDO/Municipality and uploaded on the online portal; Unique Registration Number (URN) is generated against each family after de-duplication; Smart Card distributed on the spot in camp mode or permanent enrolment centre.

Eligibility criteria: 

1. Should be permanent residents of West Bengal.
2. Should not be insured under any govt. sponsored health insurance scheme,
3. Should not receive medical allowance with the salary.

Required documents:

Aadhar Card, Identity Proof-Like Voter Card, Ration card, etc, Permanent Residence Proof.
